This paper argues that the sociology of deviance can be used to improve our understanding of some difficulties and unintended effects of health-promotion interventions designed to change risk behaviours, especially drug-taking. Firstly, many people engaged in 'risk behaviours' tend to deny the 'risky' label just as delinquents neutralise the 'deviant' label, and preventive information itself may be used by individuals in shaping risk denial. Secondly, deliberate risk-taking may be an 'innovative deviance',which is related to difficulties of conforming to the dominant 'risk culture'. Health promotion is likely to be quite ineffective if it remains wedded to the dominant risk culture and de facto contributes to the spread of it.

[fre] Les usagers des services d'hébergement ou de distribution de repas chauds constituent une population très particulière, qui cumule précarité et problèmes de santé. Les contacts avec la famille, les proches et les connaissances sont caractérisés par un cumul et non une différenciation des relations interpersonnelles. On peut distinguer les enquêtés selon que ces contacts sont fréquents ou non, quel que soit le lien de parenté éventuel entre la personne interrogée et les proches. Une relation significative apparaît entre ce cumul et la santé perçue : ceux qui ont des contacts plus fréquents avec leurs proches se jugent plus souvent en bonne ou en très bonne santé. En présence d'une pathologie chronique ou grave, ces contacts pourraient jouer le rôle de facteurs protecteurs contre le risque de dépression et qu'à ce titre, ce soit l'absence relative de contacts avec les proches et non l'absence de proches à contacter qui soit la plus préjudiciable pour la personne malade, ce qui suggère qu'il faille privilégier une interprétation faisant intervenir l'influence des rapports avec les proches sur la construction de l'identité personnelle et l'estime de soi. Un autre aspect de la relation entre santé et lien social renvoie au recours aux soins, étudié ici dans le cas particulier des soins dentaires : l'isolement relationnel s'avère en effet significativement associé à un moindre recours à ces soins. Toutefois, si le lien fréquemment exploré dans la littérature entre lien social et santé s'avère ici globalement vérifié, il convient d'y apporter une nuance. En effet, s'il est très plausible que les liens interpersonnels aient une influence bénéfique sur la santé, il importe de souligner qu'ils ne constituent pas la panacée. The ANRS-EN12-VESPA 2003 survey interviewed a national, representative sample of the HIV-positive population attending hospital outpatient services. Conducting the survey in a hospital made it easier to identify and contact HIV-positive patients, but involved various biases : the role of the physicians in choosing which patients would be interviewed, lack of time for certain categories of patients, oral and written language skills required to answer questions. The survey design enabled us to study these biases in detail. Overall, significant biases were observed, but the resulting distortions in the sample were quite small. Among the factors responsible for non-participation in the survey, those related to the patient weighed more than those linked to survey organization. Alongside the fact that persons infected through homosexual transmission were more likely to respond, which probably reflects a greater ability and willingness to participate, we also noted the importance of work and family constraints, whose effects are probably sexually differentiated, as well as the language barrier, which puts foreigners at a disadvantage, especially for the written questionnaire.

The concept of “risk culture” coined by Giddens offers an opportunity for communication of ideas between sociology and economics, because it assumes that today everyone is requested to behave as a “homo oeconomicus”. But in general a new norm encounters resistance, it has to compromise with previous norms, and it can be distorted. Thanks to data from a cross-sectional survey conducted by the French Institute for Statistics and Economic Studies, this article shows that the spread of risk culture is far from homogeneous in French society. Moreover, people who conform to risk culture (and are then closer to the “homo oeconomicus” model) are more superstitious and more prone to engage in risky behaviours when driving or during their leisure time. Normative tensions due to difficulty to conform to risk culture could explain these apparently paradoxical results.

Nowadays, experts are often said to be affected with a generalized "risquophobia", by using the notion of risk in order to blame technical innovations and business spirit. But a historical and sociological appraisal of the notion of risk leads to radically different conclusions. Firstly, the notion of risk has been built as an auxiliary for economic innovations. Secondly, contemporary societies are characterized by a "risk culture" which urge each of us to be the real entrepreneur of his own life. At last, the notion of risk can also be considered as a strategic resource which can be summoned to promote social change.
